Neck & Back Pain
Back and neck pain is the most common reason people come to Westside Chiropractic, and the complaint patients write about most in their reviews.
What the condition usually is and what good care for it looks like, from the published research and guidelines below. Then how we approach it, and what patients said afterwards.
The condition
What back and neck pain usually is
Low back pain is the leading cause of disability in the world, and for nearly everyone who has it there is no single physical cause to find. Only a small number turn out to have a fracture, an infection or a tumour behind it.[1]
Neck pain is fourth on the same list, and more than three people in ten have some in a given year. Most acute episodes settle, though close to half of people go on to have continuing pain or recurrences.[2]
New low back pain improves substantially over the first six weeks (moderate-certainty evidence); pain that has been around a long time improves much more slowly (very low-certainty evidence). That is the natural course of the problem, not evidence that any treatment caused it, and not a schedule.[3]
The standards
What good care for it looks like
Guidelines around the world agree on where care starts, and it is not with a scan or a prescription. The international position is a first approach without drugs: advice and information that help you manage it yourself, and getting back to normal activity and exercise.[4, 5]
The American College of Physicians recommends treating new back pain without drugs first — heat, massage, acupuncture or spinal manipulation — and heads its list for long-standing pain with exercise. It prints the grade of evidence beside each option, and manipulation carries the lower grade in both.[6]
“Consider manual therapy (spinal manipulation, mobilisation or soft tissue techniques such as massage) for managing low back pain with or without sciatica, but only as part of a treatment package that includes exercise.”
That is the shape of it in every guideline: hands-on treatment as one part of a plan, not the whole of it. That is the guidelines speaking, not us. Cochrane found exercise probably helps long-standing back pain compared with no treatment, while being candid that its advantage over other conservative treatments was small and, in the reviewers’ own words, not clinically important; neck strengthening has moderate-quality support from reviewers who say plainly that no high-quality evidence has been found yet.[7, 8]
Two fair questions
About scans, and about safety
For back pain with no warning signs, scanning straight away does not produce better pain or function — six randomised trials in 1,804 people found no difference. NICE advises against routine imaging outside a specialist setting, and the chiropractic profession has published the same finding about its own X-rays. A report is not an explanation either: MRI finds “degenerative change” constantly in people with no pain at all.[9, 5, 10, 2]
On safety, Cochrane notes that rare but serious adverse events exist with neck manipulation. On the stroke question, a systematic review found the evidence neither confirms nor rules out a link, and asked that consent tell patients neck movements including manipulation may increase the risk of a rare form of stroke; the largest study — 818 such strokes over more than 100 million person-years — found no excess risk with chiropractic care compared with primary care, concluding that people whose artery has already begun to tear seek help for the pain it causes shortly beforehand. Either way the rule is the same: neck pain or a headache that is sudden, severe or unlike anything before should be assessed urgently rather than treated.[11, 12, 13]
How we work
What happens when you bring it in
Every new patient starts the same way: a full medical history, an orthopedic and neurologic evaluation, and a review of any imaging where that is appropriate. Plans pair hands-on treatment — full manual technique, or lighter drop-table and instrument work chosen from your preference and the examination — with stretches and exercises for the complaint you came in with, which nearly every new patient leaves with.
A full office visit.
Your first visit will be roughly 45-60 minutes in duration and involve the following: full medical history, orthopedic & neurologic evaluation, review of imaging (when appropriate), additional referrals & co-management (when needed), and your first homework: lifestyle changes, rehab exercises and pain education to start at home.
Treatment on the same day
Almost always, we are able to treat on the first visit. If we find any red flags during your examination or another need for a referral prior to treatment, we will let you know and schedule accordingly.

One of the biggest differences you’ll notice about our practice is that we are upfront and honest about what we can and cannot help you with. This means that we’ll make sure you’re being co-managed by all of the right providers in all specialties.

Worth knowing
When to get urgent help
Almost all back and neck pain is not dangerous. These are the exceptions worth knowing.
Now — emergency department, or call 911
- New back pain with any of these: trouble starting to pee, or not feeling the flow; numbness around the genitals, the back passage or the area you sit on; new weakness or numbness in both legs; or new bowel or sexual changes. This is cauda equina syndrome, and it needs emergency diagnosis and surgery to prevent permanent damage.[14, 15]
- Sudden numbness or weakness of the face, arm or leg on one side; sudden confusion or trouble speaking; sudden trouble seeing or walking; or a sudden severe headache with no known cause. Call 911 — every minute counts.[16]
Soon — get it looked at properly
- Numb or clumsy hands together with unsteadiness on your feet, dropping things, frequent falls or bladder changes — especially over 50. That points at pressure on the spinal cord rather than a simple neck problem, and it is commonly missed for years.[17]
- Back pain after a significant injury, or in someone on steroids or immune-suppressing medicines; with a history of cancer or unexplained weight loss; or pain that is there at night and at rest rather than with movement.[18]
These are prompts to look more carefully, not a checklist to diagnose yourself with. Having one does not mean something is seriously wrong — it means it is worth someone checking.[18]
